in short

To request a block on a lost or stolen phone in India, you use the Sanchar Saathi (CEIR) form. It needs three things ready before you start: a re-issued SIM for the number that was in the phone (the one-time code goes to it), a police complaint, and an identity document. The form gives you a Request ID — keep it: the status can be checked with it or with the police complaint number, and the unblock form asks for it if the phone is found.

Below: what to prepare, each part of the form, and what happens after you submit.

The form, section by section what each part asks for

Device information

Mobile Number 1 (the number in the lost phone, re-issued), Mobile Number 2 if any, IMEI 1 and 2 if available, device brand, model, price, and an invoice if you have one.

Lost information

Where and when it was lost, the State/UT, district and police station, the police complaint number, and the complaint copy.

Owner and identity

Owner name and address (age, gender and email are optional), the identity type, its number and an upload.

OTP, declaration, Request ID

Enter the one-time code sent to Mobile Number 1 and accept the declaration that the details are true. Keep the Request ID you get.

Limits: when the online form won't work

  • Requests are accepted only if the loss date is on or after the service's launch date for your State/UT; the form shows the launch dates.
  • A new phone that was never used with a SIM: the portal tells you to contact its CEIR District Nodal police officer with a copy of the FIR or police complaint containing the IMEI.
  • The one-time code goes to Mobile Number 1, the number that was in the lost phone — so that SIM has to be re-issued and receiving SMS.
  • The Request ID is 18 digits (for example 202412132300527123); without it, recover it before you try to unblock.
  • The form ends with a declaration: false or misleading details can make you liable.

Scams after a theft

  • Apple will never contact you to say your iPhone has been found; never share your passcode, passwords or verification codes with anyone who asks.
  • If money was taken through your phone, that is a cybercrime: call the helpline 1930 or report it on cybercrime.gov.in.

Keep the police complaint copy and the Request ID together: the status check needs the Request ID or the complaint number, and the unblock form asks for the Request ID and the complaint.

Questions about the block request

Can I block my phone without a police complaint?

No. The block form asks for the police station, the police complaint number and a copy of the complaint.

Why do I need a re-issued SIM?

The one-time code for the request is sent to the number that was in the lost phone, so that number must be active again on a re-issued SIM with SMS working.

Can I use Aadhaar as ID?

Yes, as a masked Aadhaar — a document where the first 8 digits are hidden (XXXX XXXX 1234). PAN Card, Voter ID, Driving License and other identity documents are also options.

Do I need the IMEI and the bill?

The IMEI is asked for if available, and the purchase invoice is optional. The device brand and price are required fields.

I lost my Request ID. What now?

Use the portal's «Forgot Request ID» option: it sends a one-time code to the mobile number you gave while blocking.

How do I check the status?

On the status page, enter the 18-digit Request ID or the police complaint number — including a complaint number from your State/UT police portal.

The phone was found. How do I unblock it?

Use the unblock form with the 18-digit Request ID, a police complaint number and copy, and the reason: recovered by police, found by self, blocked by mistake, or other.

Suggested first steps. Nearby, or likely stolen?

If it may be nearby, play a sound. If it may be stolen, secure it first in your own Google or Apple account, then check when its location was last updated. Not expecting it back? Ask your carrier to suspend the SIM.
